Everything came back 'normal', so why don't you feel it? Normal vs functional blood testing

If you've had bloodwork done and been told it's ‘all normal’ — while still feeling exhausted, foggy, or just not yourself — you might wonder what’s going on, feel confused or uncertain about what to do next.

There's a real, well-documented gap between what a lab calls normal and what your body needs to actually function well, and almost nobody explains it before sending you home.

what ‘normal range’ actually means

A lab reference range isn't a measure of health. It's a statistical range — usually built from wherever roughly 95% of a tested population sits, regardless of whether that population was thriving or just not yet diagnostically sick. If a marker is a bit low or a bit high but still inside that band, it gets marked ‘normal,’ even if a meaningful number of people sitting at the edges of that range don't feel well at all.

That's the gap between normal and optimal. Normal tells you where most people land. Optimal tells you where people tend to actually feel good. They are not the same bar, and a result can clear the first without coming anywhere near the second.

why this leads to dismissal

Standard practice is built around ruling out disease and satisfying insurance companies or government criteria (eg Medicare, NHS), not optimising function or acting preventatively.

The problem is that a lot of people are sitting in the space between ‘not diseased’ and ‘actually well,’ and the standard range has no language for that space at all.

Here's what that looks like with some of the markers I see dismissed most commonly.

Vitamin D

Vitamin D is actually a hormone, not just a vitamin, and it's involved in far more than bone health — immune regulation and mood among them. Standard ranges mark "sufficient" anywhere above about 20-30 ng/mL, a cutoff set to prevent bone disease (rickets, osteomalacia).

Functional ranges aimed at broader benefits are usually placed meaningfully higher, commonly 50-80 ng/mL. That gap is exactly where a lot of people sit: technically sufficient, still fatigued, still catching everything going around.

Ferritin

Ferritin measures your iron stores, and most labs call anything above roughly 10-15 ng/mL ‘normal’ — because that's the cutoff used to diagnose iron-deficiency anaemia. It was never meant to define where you'd feel well.

Iron stores in the low-normal range are commonly associated with fatigue, hair thinning, and poor exercise tolerance, even in people who are nowhere near anaemic. A ferritin of 15 and a ferritin of 90 can both say "normal" on the same lab report and mean very different things for how you feel.

ESR

Erythrocyte sedimentation rate is a marker of inflammation somewhere in the body. It doesn't say where, and it doesn't say why — which is exactly why a mildly elevated result gets waved off as ‘nonspecific.’

But nonspecific doesn't mean unimportant. It means the marker is doing its job (flagging that something's inflamed) and the interpretation work hasn't been done yet. A raised ESR is a clue meant to be followed alongside your symptoms and other markers, not filed away because it doesn't diagnose anything by itself.

vitamin B12

Standard B12 testing measures total serum B12 — including the portion that's bound and not actually usable by your cells. You can have a ‘normal’ total B12 and still be functionally deficient where it counts. Fatigue, brain fog, and tingling can all persist despite a "fine" B12 result, particularly where absorption is the real issue (low stomach acid, gut inflammation, certain medications).

Better functional markers, like methylmalonic acid (MMA) and active B12 (holotranscobalamin), reflect what's actually usable rather than just what's circulating.

the pattern underneath all of this

None of these examples are really about one marker. They're about what happens when a system built to catch disease is asked to explain wellness — it wasn't designed for that question, so results in the space between ‘sick’ and ‘well’ tend to get read as reassurance rather than as information.

A single test is a snapshot. The pattern across several markers, alongside your actual symptoms and history, is usually where the real answer sits.
